Please keep an accurate record of all of your quail
hunting efforts for the season on the survey form provided. This is for wild
quail hunts only. Please do not record hunt information for hunts for pen
raised or released birds. All information regarding hunters and locations
are confidential. Your honesty and cooperation are greatly appreciated.

Record all hunts on a separate line, whether successful or not.
All hunts are important!
2.
Record the number of hours hunted to the nearest ¼ hour.
3.
If you hunt in more than one county on the same day, list all
counties and the corresponding information on different lines.
4.
Please keep the survey in a convenient location (on the refrigerator,
in your glove box, or with your hunting gear) to record results after each
hunt.
5.
Only one hunter in a party should complete this form. A party is
defined here as a group of hunters who hunt together and whose actions
likely influence success of other hunters in the group. Two or more
individuals who hunt in the same general area but do not influence each
other’s chance of seeing game should not consider themselves as a party, but
should instead fill out separate forms.
6.
If you need more than the space provided you can use more than one
survey form. Just be sure to send all of your individual forms in
together.
